The Wall of Flesh is the toughest challenge of normal mode in Terraria. Its defeat initiates Hard Mode in your world, where greater challenges await. This guide to defeating the boss will describe the battle, suggest the armor you should have to be safe, and recommend a good "arena" setup for defeating it.
Gear Prep
You should have at least the Meteor Armor or, preferably, the set you get from Crimson/Corruption bosses in order to kill the Wall of Flesh. Meteor Armor with the Space Gun is perfectly viable, so long as you have 300+ life. Weapons of that level are also recommended - the type is up to you. It is much easier to fight ranged, but broadswords can hit the wall of flesh even when you're running away from it because of their arc going over your character's head. Regeneration and Ironskin Potions are recommended, as well as Well Fed. If you don't already farm for potions, see my guides to Plants and how to make Potions.
Setting Up the Underworld
The best thing you can do for yourself is to build a platform of Ash blocks (so Fireblossom can spawn) across a large area, just a bit above the lava. Think 6+ screens, maybe more. You should have plenty of ash if you've farmed Hellstone. As to where you should do this depends upon where you entered the Underworld. The edge of the map you are closest to is where the boss will spawn. It will then move the opposite direction. If you're close to the eastern side, it will move slowly west, forcing you to stay ahead of it.
Setting up this ash bridge can take some time, as you may need to break some of the Underworld structures in the process. You need to ensure you have room to dodge and no stray blocks to get caught on. Of course, Underworld monsters will also be attacking you, but they may drop useful items, and you'll find plenty of healing potions in the pots you smash. Place Campfires along this bridge, using your buff panel to see when you're out of range for the regen buff. Go a bit further and plant another. Imagine them extending an aura in a circle, as they do, meaning you don't need to place one as soon as the buff disappears.
Voodoo Demon & Summoning
Another aspect of preparing for this fight is to avoid accidentally killing the Voodoo Demon as it flies over lava. The Demon will spawn once in a while, and if killed over lava it will drop the Guide Voodoo Doll in and initiate the boss fight. You can pick it up so long as you make the kill over land or a platform, then throw it in yourself to summon the Wall of Flesh when you're ready. Doing this will kill the Guide NPC. Do not do this if he's already dead, meaning you must wait until he respawns if you want to farm the Wall - it'll just waste a Voodoo Doll.
The battle itself is fairly straightforward. The wall will move across the map as described earlier, while firing eye lasers at you. Falling behind will cause you to get thrown back in front of it, taking some damage. So it's best to stay in front. It is important to note that you will die if attempting to escape from this fight, due to a curse the boss places on you. If you use a recall mirror or potion, you'll die as soon as you spawn in town. Your gold/gear will not be in hell, it's just not an ideal 'escape'.
The wall features these mouths that extend out, known as the Hungry. Killing them will cause them to detach and fly at you. They drop hearts, so use that to your advantage and try to kill them when they're close enough you can grab the healing. In Expert mode, they will respawn, but for normal they can be completely eliminated. It is important to do so, because they will block your damage against the wall - either preventing you from getting into melee range without damage or blocking arrows. Jester's Arrows are excellent for clearing them, as is the Space Gun - both pierce. However, you can't really beat a fiery greatsword slashing through them all and setting them ablaze. They're vulnerable to debuffs like On Fire.
The boss features three components - a mouth and two eyes. The eyes are what will shoot lasers, which are not that damaging. The most damage comes from contact with it. All damage the wall deals will increase a bit as it gets damaged, with its speed also ramping up - the reasoning for the long bridge, so that you do not have to worry about lava. The mouth is not your target, though all three share the same health pool of 8,000. The mouth has more defense, while the eyes have zero. Melee players can kite the wall and hit the bottom eye, ranged can choose between them. Keep hammering at the eye, occasionally jump over a laser and keep moving away from the wall and you can slowly earn your victory. It may take an extra try, because the boss is intimidating given its size, and the sense of pressure from the battle. Many players will encounter it on accident by making the Voodoo Demon drop the doll.. but with preparation, good gear, and a little skill, this wall is far from the hardest challenge in Terraria.
The Wall of Flesh will explode, leaving behind a box that you can mine into in order to get your loot. This is to protect the loot from falling into lava. The 100% drop is the Pwnhammer, which is used to break Demon Altars to bless your world with new ore types. The other weapons are not very important, but he's worth farming in order to get one of the Emblems. They are used to make the powerful Avenger Emblem later, which boosts all damage by 12%. Console/Mobile players (at time of writing) need to farm all three available types in order to make the Avenger Emblem, so they will have to farm the wall. It is much easier in Hard Mode gear, it just takes time for the Guide to respawn each time.
